By submitting this Know Me File, I understand and acknowledge that:

  • Submitting a Know Me File is voluntary and does not replace calling 911 or contacting emergency services.
  • The City of Pocatello and the Pocatello Police Department do not guarantee that the information will be reviewed, relied upon, or affect the response to any call for service.
  • Responders may not know who is involved when a call is received. If an individual’s identity is not known at the time of dispatch, information from the Know Me File may not be communicated to officers prior to their arrival.
  • Emergency Communication Officers will make every reasonable effort to share information from the Know Me File with responding personnel when it is applicable and available, but there is no guarantee that this will occur in every situation.
  • The information provided to this program is advisory only and does not guarantee any specific outcomes or results.

Public Records Notice
Information submitted to the Vulnerable Person KNOW ME File is a government record and may be subject to disclosure under Idaho public records law.

Verification and Record Updates
To keep information accurate, the KNOW ME File team will attempt to contact the authorized registrant or caregiver about once a year to confirm or update the file. Multiple attempts may be made to reach you. If the information cannot be verified within two years, the record will be removed from active status to prevent outdated information from being used. Removal does not prevent future enrollment. A new form may be submitted at any time.
